Schedule
AM shifts, 5 × 8 hour shifts (40 hours/week), Monday to Friday.
Job Description
We are seeking a dedicated Home Hospice Nurse to provide high-quality care to our patients. The ideal candidate will operate within the coverage area of approximately a 60-mile radius and may assist in various territories as needed.
Responsibilities
  • Conduct patient visits and provide necessary assessments and care.
  • Meet productivity expectations of 24-30 visits per week within a structured 40-hour work week.
  • Document care and maintain accurate records in accordance with agency and regulatory standards.
  • Collaborate with interdisciplinary team members to ensure comprehensive patient care.
  • Participate in ongoing training and professional development as required.
Required Experience / Certifications / Licensure
  • Previous hospice experience is required.
  • Experience with HomeCare HomeBase (HCHB) is required; alternative experience may be considered at a reduced rate during the training period.
  • A valid Driver's License and auto insurance are mandatory.
