Position Overview

The selected candidate will deliver direct patient care as a Nurse Practitioner at our Center, adhering to established protocols, policies, and recognized standards of care. This role operates within the approved scope and credentials as determined by our credentialing committee. The Nurse Practitioner will follow established nursing standards and provide specific care instructions to nursing staff and other team members.

Key Responsibilities
  • Conduct thorough and timely assessments, evaluations, and treatments for patients ranging from six months to geriatric age.
  • Demonstrate the ability to accurately interpret electrocardiograms and laboratory test results.
  • Deliver precise diagnoses and manage care for patients with chronic conditions such as hypertension, diabetes, and asthma.
  • Encourage disease prevention through effective risk assessments, coaching, and counseling.
  • Complete Annual Medicare Wellness physicals promptly and accurately, in line with organizational policies and Medicare guidelines.
  • Handle unexpected medical emergencies with competence.
  • Communicate professionally and effectively with patients, colleagues, and healthcare providers.
  • Exhibit a dynamic, compassionate, and efficient work ethic, complemented by strong organizational and customer service skills.
  • Perform additional duties as assigned, which may include working at other Primary Care locations as needed.
Qualifications
  • 2 to 4 years of relevant experience.
  • Experience in Family Medicine is preferred.
Competencies and Skills
  • Strong clinical assessment, diagnostic, and decision-making abilities.
  • Proficient in ordering and interpreting diagnostic tests.
  • Excellent skills in patient education and counseling.
  • Strong communication and teamwork capabilities to collaborate effectively with physicians and multidisciplinary teams.
  • Ability to manage a high volume of patients efficiently.
  • Familiarity with EPIC electronic health records (EHR) and medical documentation.
  • Commitment to evidence-based practices and ongoing professional development.
Credentials
  • Valid and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the practicing state.
  • Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Physician Assistant (PA-C).
  • Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
  • DEA registration for prescribing medications, if applicable.
Education
  • Master's degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program is essential.
