Job description

Position Overview

Functions within the nursing department to coordinate the delivery of appropriate and timely care for patients, promoting quality and cost-effective health care outcomes. Responsible for active management of patients, collaborating with medical providers and interdisciplinary teams to establish individualized medical goals.

Key Responsibilities

  • Manages panel acute/chronic care needs and health maintenance via EHR review and patient outreach

  • Collaborates with medical providers and interdisciplinary team members to establish individualized medical goals

  • Coordinates multi-disciplinary care conferences for high-risk or complex patients

  • Facilitates patient referrals within and outside facilities; tracks progress and outcomes

  • Performs staff nursing duties including assessment, documentation, education, and medication administration

  • Serves as primary link for case managers and discharge coordinators in care coordination

  • Specialty Clinic: manages referrals/waitlists, coordinates clinics, and serves as primary nurse for specialty visits

  • OB Case Management: intake, tracking, education, outreach, and postpartum follow-up

  • VTC/ETT coordination with specialty providers and regional facilities

  • Participates in triage role and responds to emergency situations

Education, Certifications & Licensure

  • Graduate of accredited nursing school; BSN preferred

  • Current full unrestricted RN license in Alaska or another U.S. state (must apply for AK license before start)

  • Basic Life Support (BLS) - American Heart Association

  • National Case Management certification preferred

  • Clinical Competency required within 3 months of hire and every three years

Experience Required

  • 1+ year general nursing with 6 months at SEARHC outpatient clinic, OR

  • 2+ years general nursing with minimum 1 year in ambulatory setting or 1 year case management experience
