Shift: Monday - Friday | 8:00 AM - 4:30 PM
As a Physical Therapist (PT) on our Home Health team, you'll bring expert care and support to patients where they feel most comfortable-their own homes. You'll use your clinical skills, creativity, and compassion to help individuals regain strength, mobility, and independence. Collaborating closely with physicians and the interdisciplinary care team, you'll help develop and deliver personalized therapy plans that are rooted in best practices and focused on achieving meaningful outcomes. Responsibilities
• Makes visits to the homes of patients requiring home health physical therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
• Performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures.
• Evaluates and assesses the patient's level of functioning by applying appropriate tests.
• Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.
• Initiates diagnostic, preventative and rehabilitative procedures as appropriate to the patient's care delivery
Qualifications
Education:
  • Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.
  • Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.

Licensure/Certifications:
  • Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:
  • One year experience caring for similar patient population as to be assigned.

Other:
  • Use of usual and customary equipment used to perform essential functions of the position.
