Home Care RN Inc. is a fast-growing, clinician-led home health agency proudly based in Colorado. We're locally owned, nurse-led, and deeply committed to providing daily skilled nursing visits. We are committed to serving a Medicaid-focused population that society often ignores or forgets, a level of frequency and quality that sets us apart in the industry.
Position Summary:
Entry-level Community Liaison who builds relationships with patients, families, referral sources, and community partners to support referrals, patient access, care coordination, and positive patient experience. Acts as the agency?s local representative to increase awareness of services, assist with intake/initial scheduling, and support retention and outcomes.
Key Responsibilities:

  • Represent the agency at community events, health fairs, senior centers, clinics, long-term care facilities, and physician offices.
  • Develop and maintain relationships with referral sources (hospitals, clinics, physician practices, social workers, care managers, skilled nursing facilities, and community organizations).
  • Conduct outreach calls and follow-ups to referral sources to ensure timely patient referrals and receive feedback.
  • Assist with intake processes: gather basic patient information, verify insurance/benefits, schedule initial home visits or assessments, and coordinate with clinical intake staff.
  • Provide patient/family education on home health services, eligibility, and what to expect during care.
  • Track and document outreach activity, referral source interactions, leads, and outcomes in the agency CRM/EHR per policy.
  • Escalate clinical, eligibility, or scheduling issues to intake/clinical supervisors promptly.
  • Support discharge planning and transitions by communicating with hospitals and care managers to facilitate smooth home transitions.
  • Prepare outreach materials (brochures, flyers) and assist in deploying marketing collateral.
  • Assist with basic data collection and reporting for outreach performance metrics.
  • Maintain professionalism, confidentiality, and compliance with HIPAA and agency policies.


Required Qualifications:

  • High school diploma or GED required. Associate degree or coursework in healthcare administration, social services, public health, or related field preferred.
  • 0?2 years customer service, community outreach, sales, care coordination, or healthcare-related experience preferred.
  • Valid driver?s license and reliable transportation for community visits.


Skills and Attributes:

  • Strong verbal and written communication skills; comfortable presenting and networking in community settings.
  • Empathetic, patient-centered approach and ability to interact professionally with diverse populations (older adults, caregivers, clinicians).
  • Basic organizational skills: scheduling, follow-up, and time management.
  • Ability to learn agency services, eligibility rules, and referral pathways quickly.
  • Team player with ability to work independently in the field.


Physical Requirements and Work Environment:

  • Ability to travel locally and attend events; may require standing for extended periods.
  • Ability to enter patient homes and long-term care facilities; must follow infection control and security policies.
