Job description

The Manager of Practice Success & Transformation is the primary operational relationship owner for a portfolio of Axil Health physician-practice and health-system partners. This individual works directly with practice administrators, office managers, physicians, providers and Axil clinical teams to ensure each partnership is successfully implemented, consistently supported and producing measurable clinical, operational and financial value.
The ideal candidate understands how medical practices operate, can earn credibility with physicians and practice leaders, and is comfortable using performance data to identify problems, facilitate decisions and drive accountability.
This is not a traditional customer-support position. It is a practice-facing leadership role responsible for partnership success, workflow transformation, program adoption, performance improvement, retention and responsible growth.
Primary Responsibilities
Practice Relationship Ownership & Performance Management
  • Serve as the primary nonclinical point of contact for assigned physician practices and health-system partners.
  • Develop trusted relationships with practice administrators, office managers, physicians, providers and operational leaders.
  • Maintain a deep understanding of each practice's goals, workflows, staffing structure, patient population and financial priorities.
  • Identify practices at risk of dissatisfaction, underperformance or disengagement and lead a structured recovery plan.
  • Conduct a structured monthly performance meeting with each assigned practice, presenting a practice specific scorecard and maintaining a written action plan with responsible owners and completion dates.
  • Conduct quarterly strategic reviews with practice leadership and physician champions, including renewal and contract-review conversations as needed.
  • Identify expansion opportunities within existing partnerships and partner with Axil growth leadership to assess and pursue them.

Practice Transformation and Adoption
  • Assess current practice workflows and identify opportunities to improve patient identification, enrollment, care-team collaboration, documentation, provider communication and charge capture.
  • Develop jointly agreed-upon Practice Success Plans for new and existing partners, and support optimization of CCM, APCM, PCM, RPM, TCM, AWV and other Axil Health programs.
  • Integrate Axil services into existing EHR and practice operational workflows, coordinating Axil Clinical operations, billing, technology and leadership teams to resolve barriers.
  • Train office managers, providers and practice staff on Axil Health programs, workflows, responsibilities and technology.
  • Track adoption of VitalSmart, practice workflows and agreed-upon operational processes.

  • Strong practice retention, satisfaction and relationship health across your portfolio
  • Consistent adoption of Axil programs (CCM, APCM, PCM, RPM, TCM, AWV) and the VitalSmart platform
  • Reliable, on-time completion of practice reviews with clear, closed out action plans.

Qualifications
  • Bachelor's degree in healthcare administration, business, public health, nursing or a related field, or equivalent relevant experience.
  • 3 years of experience in physician-practice operations, ambulatory operations, population health, value-based care, provider engagement or healthcare customer success. Including direct experience working with practice administrators, office managers and physicians.
  • Strong understanding of medical-practice workflows and EHR-enabled operations with the ability to interpret operational, clinical and financial performance data.
  • Strong presentation, facilitation and executive-communication skills with a track record of identifying workflow problems and leading practical improvement plans.
  • Demonstrated ability to manage multiple accounts, projects and deadlines simultaneously.
  • Ability to travel regularly to assigned physician practices.

Preferred Experience
  • Previous experience as a medical-practice manager, practice administrator, ACO practice-transformation specialist or ambulatory-operations manager.
  • Experience with CCM, APCM, PCM, RPM, TCM, AWVs or other population-health programs.
  • Familiarity with value-based care, ACOs, Medicare programs and quality-performance measures.
  • Experience with Epic, eClinicalWorks, athenahealth, Veradigm, NextGen or similar EHR platforms.
  • Experience working with primary care, cardiology, endocrinology, pulmonology or nephrology practices.
  • Familiarity with healthcare billing, revenue-cycle processes and clinical documentation.
