Job description

Wound Care Nurse (LPN/RN)

Ascend Medical is hiring experienced Wound Care Nurses (LPN or RN) to support our growing wound care program across Northeast Georgia. This is a traveling, home to home role supporting multiple regions. The ideal candidate brings strong wound care expertise, comfort working independently, and a patient-first mindset.

You will assess, treat, and manage complex wounds while collaborating with providers and clinical teams to improve healing outcomes and patient quality of life.

Schedule: Monday, Wednesday, Friday (3 days/week) Territory: Buford, Braselton, Lawrenceville, Snellville, Loganville and surrounding Northeast Georgia. Employment Type: 1099 Independent Contractor (Part-time, Full-Time)

Key Responsibilities
  • Conduct comprehensive wound and patient health assessments
  • Develop and implement individualized wound care plans
  • Perform wound treatments, dressing changes, and topical applications
  • Monitor healing progress and identify signs of infection or complications
  • Educate patients and families on wound care and prevention
  • Collaborate with providers and clinical staff to coordinate care
  • Document assessments, treatments, and progress in the EMR
  • Travel between designated clinic locations within the territory
  • Stay current on wound care best practices and clinical guidelines

Qualifications & Requirements
  • Active Georgia LPN or RN license (required)
  • Minimum of 2 years of wound care experience (required)
  • Home health or clinic-based wound care experience strongly preferred
  • Experience with Tabora EMR preferred
  • Strong knowledge of wound healing, infection prevention, and DME related to wound care
  • Reliable transportation and willingness to travel between clinics
  • Strong communication, documentation, and patient education skills
  • Compassionate, flexible, and committed to high-quality care
  • Experience with pediatric patients, tube feeding, or med-surg nursing is a plus
