Providing Home-Based Disability Support Services Across Connecticut.

Team Members support clients with:
  • All aspects of home and community life.
  • Ensuring consistent safety.
  • Providing a healthy and professional friendship.
  • Personal hygiene and dressing.
  • Scheduling, attending and reporting on medical and therapeutic appointments.
  • Shopping, cooking, and cleaning with the client.
  • Organizing consistent exercise, socialization, and entertainment.

Clients live alone or with family. Clients and care plans are directed and monitored by DSS, DMHAS, BRS, DDS, Legal Conservators, CCCI, SWCAA, WCAAA, and care management access agencies for daily efficiency.

Requirements:
  • Pre-employment drug screen and criminal background check.
  • Candidates should have one or two years of experience working with and/or providing for individuals who live at home and have had one of the following: stroke, concussions, brain injury, TBI.
  • Present a letter from a person and/or employer verifying experience.
  • Car, driver’s license and insurance.
  • High-school and/or GED graduate.
Pay Rate:
Dependent on experience and client needs.
