The HUB Case Manager is a highly visible, relationship-driven role that combines nursing expertise, workers' compensation knowledge, employee education, and operational support. This position plays a key role in helping employees successfully navigate their recovery and return-to-work journey while supporting leaders and stakeholders throughout the process.

We answer questions, provide guidance, and help reduce uncertainty during a difficult time."

You help employees understand workers' compensation processes, recovery expectations, treatment plans, and return-to-work requirements while also partnering with operational leaders to solve problems and support successful outcomes."
"As a HUB Case Manager, you become the local subject matter expert for workers' compensation and return-to-work programs, collaborating closely with leaders, labor partners, timekeeping, and other stakeholders to help employees return to work safely and successfully."

- Provide employee outreach, education, and support regarding recovery, workers' compensation processes, return-to-work expectations, and available resources.
- Review work restrictions and identify suitable modified-duty opportunities in partnership with operational leaders and stakeholders.
- Serve as a subject matter expert on workers' compensation, disability management, return-to-work programs, and employee work restrictions.
- Maintain ongoing communication with employees and stakeholders to address barriers and facilitate successful return-to-work outcomes.
- Track employee absences, case milestones, and return-to-work activities while maintaining accurate documentation across multiple systems.
- Manage a caseload and prepare reports, analyses, and recommendations for leadership regarding employee restrictions, absences, and program effectiveness.
Qualifications
- Education: Associates Degree or Bachelor’s Degree in Nursing or related field.
- Experience:
- 2+ years of clinical, case management, disability management, absence management, workers' compensation, occupational health, or return-to-work experience preferred.
- Experience coordinating return-to-work programs, managing work restrictions, transitional duty assignments, employee absences, or workers' compensation cases strongly preferred.
- Backgrounds such as Occupational Health with absence management responsibilities, Certified Disability Management Specialist (CDMS), workers' compensation adjusting, case management, leave administration, or Human Resources-related case management are highly desirable.
- Skills: Ability to effectively collaborate with employees, physicians/providers, operational leaders, Human Resources, workers' compensation stakeholders, and customers.
- Strong understanding of work restrictions, modified duty programs, return-to-work processes, and disability/absence management.
- Ability to independently manage a caseload while prioritizing multiple responsibilities and deadlines.
- Advanced computer proficiency with the ability to navigate multiple systems simultaneously and create, maintain, and analyze spreadsheets and reports.
- Proficient in Microsoft Excel, Word, Outlook, and other case management or tracking systems.
- Strong organizational, analytical, verbal, and written communication skills.
- Ability to track employee status, monitor return-to-work milestones, and maintain accurate documentation throughout the case management process.Active Illinois Registered Nurse (RN) license required and must be in good standing.
- Travel: Must have reliable transportation to drive to the onsite location.
