Job description

Role Overview As a Wellness Nurse (LPN/LVN), you support resident health, monitor care needs, and ensure safe medication practices and clinical compliance across two senior living community locations.
Key Responsibilities
  • Conduct wellness assessments and monitor resident health changes
  • Support medication management and perform MAR/TAR audits
  • Communicate with families, physicians, and care teams
  • Monitor incidents, falls, and follow-up care
  • Maintain documentation and compliance with regulations
  • Support infection control and clinical quality initiatives
  • Train and support care team members

Required Qualifications
  • Active LPN or LVN license
  • 1+ year of experience in a senior living or clinical setting
  • Strong assessment, documentation, and problem-solving skills
  • Knowledge of medication administration and compliance
  • Excellent communication skills

Preferred Qualifications
  • Previous experience in a Senior Living community

Schedule & Location Details
  • Hours: Monday-Friday, 8:00 AM - 5:00 PM (1-hour lunch)
  • Location Workstyle: Onsite multi-site rotation. Works 3 days onsite at one location and 2 days onsite at a second location, rotating the schedule every other week (3 days / 2 days).
  • Languages Required: None / Standard
