Job description

Theoria Medical is seeking a physician leader to serve as a Regional Medical Director (RMD), providing strategic oversight across a portfolio of post-acute care and senior living communities. This role supports skilled nursing, long-term care, assisted living, and independent living settings while overseeing facility-level Medical Directors and provider teams.

Working in a dyad leadership model alongside Regional Directors of Operations and Vice Presidents of Operations, the RMD drives clinical excellence, quality outcomes, provider performance, and value-based care initiatives throughout the region. The role also serves as a key partner to facilities, hospitals, and healthcare systems, helping strengthen relationships and advance organizational goals.

What You'll Do
Clinical Leadership

  • Provide oversight, mentorship, and support to Medical Directors, Attending Physicians, and Advanced Practice Providers across the region.
  • Establish and maintain clinical standards to promote quality, compliance, and exceptional patient outcomes.
  • Support provider recruitment, onboarding, engagement, and retention initiatives.

Quality & Performance Improvement

  • Lead initiatives focused on reducing hospital readmissions, improving transitions of care, and enhancing patient outcomes. 
  • Drive performance related to value-based care programs, HCC documentation, risk adjustment, and care gap closure. 
  • Promote evidence-based medicine and High Reliability Organization (HRO) principles throughout assigned markets.

Operational Partnership

  • Partner closely with Regional Directors of Operations and Vice Presidents of Operations in a dyad leadership model to drive regional clinical and operational performance.
  • Partner with operational leadership to align clinical and business objectives.
  • Support initiatives that improve productivity, census growth, care coordination, and operational efficiency. 
  • Utilize data and analytics to identify opportunities and implement performance improvement strategies. 

Growth & Strategic Partnerships

  • Collaborate with hospitals, health systems, and referring providers to strengthen post-acute care partnerships.
  • Assist in developing specialized clinical programs that support patient recovery and enhance organizational growth.
  • Support regional growth initiatives through physician leadership, hospital partnerships, and development of innovative clinical programs.
  • Serve as a physician ambassador for Theoria Medical throughout the assigned region.


Your Qualifications
Required

  • MD or DO from an accredited medical school
  • Board Certification in Internal Medicine, Family Medicine, or Geriatrics
  • Active and unrestricted medical license in applicable state(s)
  • Active DEA registration
  • Minimum of five (5) years of physician leadership experience in post-acute care, senior living, long-term care, or related healthcare environments

Preferred

  • Multi-site physician leadership experience
  • Experience leading value-based care, population health, HCC/risk adjustment, or readmission reduction initiatives
  • Demonstrated success improving quality, operational, and financial performance metrics
  • Experience leading change management and quality improvement initiatives

Work Environment

  • Full-time exempt position
  • Approximately 80% clinical responsibilities and 20% administrative leadership responsibilities
  • Regional travel required (approximately 20%) to support facilities, provider engagement, quality initiatives, and strategic partnerships
