Job description

Pro Care by Abound Health is seeking a compassionate and organized Care Coordinator to join our growing team in Farmington, Michigan. Reporting to the Program Director, you will help individuals receiving Michigan Waiver Services live more independently and remain connected to the support they need. This is an opportunity to build meaningful relationships, lead direct care professionals, and make a visible difference for clients and families every day.
Key Responsibilities:
  • Build trusted relationships with clients, families, direct care staff, and community partners.
  • Coordinate individualized services that support each client's goals, independence, and quality of life.
  • Supervise, coach, and support direct care staff.
  • Develop client goals and interventions and maintain them in OnTarget.
  • Coordinate staff schedules and backup coverage to provide consistent, reliable support.
  • Connect clients and families with helpful community services and resources.
  • Maintain accurate records and ensure services meet Michigan waiver, IPOS, and Pro Care requirements.

Qualifications:
  • High school diploma or GED required.
  • Experience in care coordination, case management, social services, healthcare, or a related field.
  • Strong communication, organization, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Knowledge of IPOS implementation and Michigan Waiver Services is preferred.
  • Proficiency in Microsoft Office; OnTarget experience is a plus.
  • Valid driver's license, reliable transportation, and the ability to pass all required background checks.

Compensation & Benefits:
Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. In addition to base pay, total compensation may include various other forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits, which begin on your 60th day of employment.
