CASE MANAGER SUPERVISOR

POSITION OVERVIEW:

The Case Manager Supervisor will assist with the oversight and implementation of Social Services functions of the shelter according to DHS regulatory and accreditation requirements care and resources for the purpose of meeting the physical, emotional, and social needs of clients. The Case Manager Supervisor provides supervision to the Intake Services and Case Management Team and works with the Program Director/Director of Social Service to develop, implement, track, report, and evaluate measurable outcomes.

The salary for the Case Manager Supervisor is $50,000 annually based on relevant experience.

KEY ESSENTIAL FUNCTIONS:

  • Serve as mentor to Case Managers; promote their professional development.
  • Ensure Case managers maintain updated case files with necessary documentation.
  • Conduct and document one-on-one supervision of direct reporting staff to assess and improve.
  • Ensure clients have appropriate medical insurance and are referred to medical provider.
  • Work with team to ensure building standards are met and conduct unit inspections and follow-up.
  • Collect, review, sign and submit timesheets for all direct reports for PD/DSS approval.
  • Assist in organizing onsite and offsite training opportunities for professional development of staff.
  • Participate and facilitate in interdisciplinary team meetings to discuss and overcome resident barriers.
  • Delegate and monitor client-related operational responsibilities, as appropriate, to Intake and Case Management team members.
  • Oversee the timely assessment of clients upon admission, change of condition and review and update care plans.
  • Assist in developing program content to address client needs related to housing, substance abuse treatment, mental health, and employment.
  • Set realistic weekly and monthly performance goals in accordance with contract requirements with reporting staff.
  • Assist with monitoring and tracking clients housing placements, medical compliance, and Individual Service Plans.
  • Conduct case conferences with difficult to engage clients and relevant Case Manager.
  • Provide supportive counseling and advocacy for clients.
  • Liaison with medical staff and psychiatric providers (both internal and external) to ensure appropriate service delivery for clients.
  • work performance (i.e. identify training needs, professional development, etc.)
  • Assist PD/DSS with ensuring housing and performance outcomes are met on a monthly/ quarterly/ annual basis.
  • Develop and implement crisis prevention/or intervention strategies to ensure a safe and secure environment.
  • Assist with attending meetings with funders or outside agencies.
  • Assist PD/DSS with implementing a system to ensure that clients maintain their medication compliance on a weekly basis.
  • Assist with maintaining compliance with all organizational policies and legal regulations, by ensuring meticulous documentation of all activities in the department.
  • Assist in the review analysis of program and demographic client data to make program improvements.
  • Report all significant (i.e. Funder) events, and monitoring events to the Program Director/Director of Social Service.
  • Assist with planning and organizing program activities to maximize program contract and performance goals as outlined in DHS Performance Incentive Plans.
  • Communicate with other Case Manager Supervisors at other Acacia Network Shelters on issues related to client progress and best practices.
  • Assist PD/DSS with conducting periodic internal file reviews and implement Quality Assurance measures as needed to ensure quality service delivery to clients (i.e. address and investigate client complaints)
  • Submit accurate DHS and ACACIA’s monthly reports to PD for review in a timely manner.
  • Assist with troubleshooting client and direct reporting staff program problems, and make decisions in accordance with program policies, procedures, and protocols.
  • Assist with completing incident reports to relevant parties to properly and timely record any incident.
  • Performs other duties assigned by the Program Director.

REQUIREMENTS:

  • Associate degree but bachelor’s degree, preferred.
  • Minimum of five (5) years’ experience working with homeless and/or and mixed income population.
  • Specialty in substance abuse and mental health issues
  • Knowledge of crisis prevention, intervention, and resolution techniques; match such techniques to circumstance and individuals.
