Job Summary:

We are seeking a compassionate and experienced Social Worker to join our team at Mountain View Rehabilitation and Senior Living Center. As a Social Worker, you will work closely with our interdisciplinary team to provide holistic care to our residents, including those with physical and cognitive impairments. Your responsibilities will include:

  • Conducting social histories and assessments to identify resident needs and develop individualized care plans

  • Providing counseling and therapy services to residents, families, and caregivers

  • Coordinating care with other healthcare professionals and community resources

  • Facilitating group activities and events that promote socialization and engagement

  • Assisting with discharge planning and transition services

  • Maintaining accurate records and documentation

Requirements:
