We are a well-established nursing home committed to providing compassionate care and support to our residents. Our facility is known for its dedication to enhancing the quality of life for seniors and their families.

Position Overview:

We are seeking a dedicated and empathetic Social Worker to join our team. The Social Worker will play a vital role in assessing residents' psychosocial needs, providing counseling and support services, and coordinating community resources to enhance their well-being.

Social Worker Key Responsibilities:

  • Conduct psychosocial assessments of residents to identify needs, preferences, and goals.
  • Develop and implement individualized care plans in collaboration with residents, families, and interdisciplinary team members.
  • Provide counseling and emotional support to residents and families facing challenges related to aging, illness, or transitions.
  • Coordinate discharge planning and facilitate transitions to appropriate levels of care or community resources.
  • Advocate for residents' rights and ensure access to entitlements, benefits, and supportive services.
  • Collaborate with healthcare professionals, agencies, and community organizations to coordinate services and resources.
  • Maintain accurate and confidential documentation of social work interventions and resident progress.
  • Participate in interdisciplinary meetings and contribute to care planning and case management.

Social Worker Qualifications:

  • Bachelor's or Master's degree in Social Work from an accredited program.
  • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
  • Previous experience in a long-term care or healthcare setting is beneficial.
