Description
Concern Housing is seeking a Service Coordinator to be responsible for assisting supportive tenants with successful community integration by using a person-centered, strength-based approach to facilitate linkages with community resources, provide individualized counseling and support, and ensure appropriate documentation and compliance with program requirements. The Service Coordinator is an essential resource and advocate for special needs tenants.
Days/Hours: Monday- Friday; 10am - 6pm
Responsibilities Include but are not limited to:
  • Facilitate supportive tenants' linkage with chosen support services available in the community (e.g., care coordination, outpatient behavioral and physical health services, dual recovery supports, vocational services and opportunities, entitlements and financial assistance and peer supports)
  • Employ a person-centered, strength-based approach to solicit interests, aspirations, and preferences toward the development of quarterly Support Plans, Functional Assessments, and Support Plan Reviews. Meeting with residents to develop skills and review progress. Responsible for all documentation required for resident record (e.g., progress notes, discharge plans, etc.)
  • Provide individual counseling and support services to help promote skill development and tenant growth, including wellness self-management, medication self-sufficiency, housing readiness and financial competency
  • Employ a harm reduction approach to improve successful health care outcomes and monitor progress toward triple aim
  • Provide supports to ensure availability of full range of housing services, including maintenance issues
  • Attendance at weekly supervision and staff meetings
  • Other duties as assigned by the Program Director or Supervisor
  • Remain current with all Agency required compliance documents and trainings

Requirements:
Bachelor's Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years' experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
