Hours: Monday - Friday, 8:00 AM - 4:30 PM (On-call for emergencies)

Remote Work: Not Available

Position Summary:

Ready 4 Work is seeking a dedicated Case Manager for our Maryland Day Program. In this role, you will help individuals with complex mental health and/or substance abuse needs achieve rehabilitative, sobriety, and recovery goals. You will work directly with clients, providing essential services that enhance their ability to manage vocational, housing, and educational challenges. Our goal is to support clients in their recovery journey and help them live as independently as possible.

Responsibilities:

  • Complete assessments, interviews, and other required documentation for clients.
  • Provide tenancy support services and assist clients in finding and maintaining housing stability.
  • Help clients access necessary resources, including medical care, social security, Medicaid/Medicare, housing, and more.
  • Facilitate rehabilitation groups and individual meetings.
  • Document interactions and develop person-centered plans with measurable goals.
  • Collaborate with other care team members, such as therapists, psychiatrists, LPNs, and family members.
  • Provide support with daily living activities, personal and social skills, and crisis intervention.
  • Coordinate transportation to appointments and community services.
  • Perform medication monitoring and ensure updated medication lists.

Qualifications:

  • Bachelor's degree in psychology, counseling, social work, rehabilitation, or behavioral science.
  • Experience working with individuals with serious mental illness or in community support settings is preferred.
  • Must be at least 21 years of age and possess a valid driver's license with a clean driving record.
  • Ability to pass a criminal background screening.
  • Strong oral and written communication skills in English.
  • Ability to perform job functions with minimal supervision while adhering to the highest ethical standards.
  • Willingness to complete required training in NCI, CPR/First Aid, HIPPA, and other necessary certifications.

Physical Requirements:

  • Must be able to lift at least 25 pounds, navigate multiple flights of stairs, and tolerate exposure to various weather conditions.
  • Ability to travel using public or private transportation as needed.
  • Required to sit, stand, kneel, and perform a variety of movements throughout the workday.
