Job description

Intake Specialist

Location: Forest Hills, PA with extensive local travel (mileage is reimbursed at the federal rate)

Schedule: Monday-Friday 7:30 am-4:00 pm

Summary of Job Duties: The Intake Specialist conducts in-home assessments and provides information to individuals and their families who are interested in enrolling in the Community LIFE program. The Intake Specialist explains Community LIFE's benefits and services, answers questions or concerns, and completes an assessment of medical, psycho-social and environmental conditions to further identify the needs and goals of potential participants and their family caregivers. The Intake Specialist assists potential enrollees with gathering information needed for clinical and financial eligibility, works closely with Community LIFE's Medicaid Eligibility Coordinator, and monitors the intake process using a customer relations management system. The Intake Specialist introduces enrollees to the Interdisciplinary Care Team, providing background information, and coordinating an in-person orientation which occurs prior to enrollment. The Intake Specialist must have excellent interviewing, record-keeping, writing, and verbal presentation skills. Additionally, he or she must be able to represent the organization professionally to referral sources and the potential enrollee's family members and caregivers.

Required Education: Associates degree in related field. Bachelor's degree preferred.

Required Experience: Minimum of one year working with a frail or elder population. Two or more years working in community health, senior health, marketing, or enrollment for a public health, social service organization or health plan.

Required Certifications/Licensing: Valid Pennsylvania Driver's License and reliable vehicle.
