Available Shifts:(1) Night Shift - 6:30 p.m. - 7:00 a.m. (Rotating Monday - Friday)(1) Day Shift - 6:30 a.m. - 7:00 p.m. (Rotating Monday - Friday)

Summary: Registered Nurse who supervises and coordinates nursing activities in the Children's Rehabilitation Center (CRC) or Butler Adult Living Center (Butler) as assigned by the Director of Nursing. Provides direct care including personal care, medication administration, lab draws, and nursing assessments. Oversees the care of individuals served by delegating work to and monitoring the work of registered nurses (RNs), licensed practical nurses (LPNs), and certified nursing assistants (CNAs) on their shift. Coordinates overall care plans of individuals at CRC and, in conjunction with Butler Administrator, for individuals at Butler. Shares in after hours medical and/or staffing call responsibility when necessary.


Essential Duties and Responsibilities: include the following. Other duties may be assigned.


Scheduling

In the absence of the Director of Nursing, Assistant Director of Nursing and/or Nurse Unit Coordinator, the nurse Supervisor will perform the following duties:

  • Develops a nurse schedule if needed with the director of nursing assistance.
  • Ensures adequate staffing 24 hours daily including ensuring nurse availability when required to accompany individuals served to medical appointments, outings, ER trips and transports home during shift with assistance of unit coordinator and the Director of Nursing.
  • Adjusts schedules as needed for scheduled/unscheduled time off . Uses all resources available to fill gaps or holes in schedules with Unit Coordinator assistance. Consults with the Director of Nursing when encountering barriers to filling gaps or holes.
  • Takes calls on weekend call rotation when scheduled and/or when needed.
  • Trained Driver for ESA for individual served transportation. Maintains car insurance.

Clinical Responsibility:

Provides nursing care according to Departmental Standards, Nurse Practice Act and intermediate care facility for individuals with intellectual disabilities (ICF/IID) Regulations.

Performs physical assessment of individuals served at Butler and CRC as assigned by the Director of Nursing upon admission, prior to discharge, quarterly, and during acute illness.

In the absence of the Staff for Butler , completes weekday AM/PM medication pass at Butler or ensures a nurse is available to do so.

Assists with medication pass at CRC and Butler as needed.

Allows individuals served to practice medication and/or nursing goals/objectives during medication passes or provision of other nursing care. Assesses progress of individuals served in meeting individualized program plan (IPP) goals and objectives and documents accordingly.

In the absence of the Nursing Coordinator, helps ensures nursing supplies and equipment are available. Works with the LPN shift Supervisor to place supply orders weekly and to prepare and submit purchase orders in.


Care Coordination:

Collects admission data and implements a plan of care for individuals served at Butler and CRC as assigned by the Director of Nursing.

Oversees nursing responsibilities in the assigned shift at Butler and CRC to make sure all required duties are completed.

Completes Butler quarterly assessments and CRC assessments as assigned by the Director of Nursing.

Communicates with families, facility staff, physicians, other facilities, and support organizations in person and by telephone as residents' condition warrants.

Coordinates admissions, discharges or changes in room assignments with Administrator and Direct Support Professional leadership at CRC and works in conjunction with Butler Administrator to provide support for Butler admissions and discharges as needed.

When the Nurse Coordinator is unavailable, assists with coordination of medical appointments with nursing staff and community health providers for individuals served at CRC and provides support to Butler Administrator on appointment coordination as needed.


Human Resources Management:

Coordinates regular quarterly nurse staff meetings and in-services with the Director of Nursing on the assigned shift.

Addresses job performance concerns within their assigned shift and informs the Director of Nursing.


Training:

Conducts orientation and annual training of non-nursing staff as assigned.

Maintains own continuing education hours for licensure.


Leadership:

  • Coordinates nursing department functions in the absence of the Director of Nursing.
  • Evaluates nursing activities to ensure consistent quality care, staff relations and efficiency of service.
  • Reports problems that impact resident care, safety and security to the Director of Nursing, Assistant Director of Nursing, Physician and Administrator as appropriate.
  • Attends staff and mandatory organizational meetings as assigned.
  • Ensures adherence to infection control standards at all locations where nursing care is delivered.
  • Protects and promotes rights of individuals served.
  • Initiates and follows through with departmental protocol in emergency situations.
  • Familiar with organization and department policies and procedures.
  • Takes call schedule as assigned by the Director of Nursing including all of the following.
  • Staff relief call Monday through Friday. If call in are during scheduled shifts and no staff willing to cover the open spot. Works open shift if unable to find nursing coverage utilizing regular or pool staff or agency staff during the call schedule. May include occasional holidays.
  • Takes their turn in the weekend call when assigned.
  • Medical call shared with the nursing director as assigned when no RN is on duty at CRC. Reports to the facility to assist if medical questions/needs cannot be resolved by phone.

Employee has access to all client protected health information within his/her program. Employees will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer.


Supervisory Responsibilities:

Directly supervises, RNs, LPNs and CNAs. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include overseeing employee training/in-services; planning, assigning and directing work; addressing complaints and resolving problems.

Requirements

Qualifications:

Education and/or Experience:

Graduate from an accredited college or technical school of Nursing (RN). Associate degree plus four years related experience and supervisory experience required. RN with bachelor's degree plus two years related experience and supervisory experience preferred.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of an organization.


Mathematical Skills:

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.


Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.


Certificates, Licenses, Registrations:
