Position Overview
As a member of the Prevention and Diversion (P&D) team and working from a trauma-informed and client-centred care perspective, the P&D Case Manager provides short-term case management support to families from the community and those referred by other Inn programs to maintain housing stability. They work directly with families who call for assistance, in collaboration with the Triage and Housing Solutions and HERO Teams and collaboratively with other community agencies. The P&D Case Manager works in tandem with the P&D Specialist to support families seeking financial support and other resources to ensure families stabilize and maintain their housing. Using a strengths-based approach, the P&D Case Manager assesses a family's needs to determine what assistance and referrals are required to succeed in maintaining or finding new housing. A goal plan is developed with the family, and short-term case management support is offered to support sustainability. They are committed to providing excellent client service and contribute to a respectful and productive work environment.
What You'll Do
- Work with families referred to the program to assess their current housing situation, determine their financial needs and advocate with landlords to maintain housing or find alternate housing.
- Meet regularly with families to monitor their situation and progress and provide case management support to maintain family housing.
- Make/support community referrals and offer system navigation assistance to help families maintain their housing.
- Liaise with the Inn's financial team regarding financial assistance requests for families.
- Connect with landlords and other community agencies to advocate for the family's needs and facilitate financial support payment processes.
- Consult with resources and advocate as needed for families with concerns about their rental situation that conflict with their rights and legislation.
- Maintain appropriate documentation and case notes of support provided to families using established systems - internal database, HMIS, SharePoint etc.
- Participate in weekly team meetings and case consults with other Prevention and Diversion Services Teams and Management.
This position description reflects the general duties of the role but is not intended to include all duties that may be inherent in the position. The Case Manager, Prevention and Diversion, may be asked to perform other duties as required or assigned.
What You Bring to Our Mission
- A post-secondary diploma or degree in a social service field is preferred; a combination of experience and other education will be considered.
- Experience working with vulnerable populations, including families experiencing homelessness.
- Solid ability to respond to clients' complex needs in a community setting.
- Comprehensive knowledge and understanding of issues related to families and homelessness.
- Solid knowledge and experience dealing with housing-related issues; prevention and diversion principles and practices are assets.
- Knowledge of housing and income-focused community resources and supports.
- Understanding of the Housing First, Harm Reduction and Trauma Informed Care practice models.
- Strong interpersonal and communication skills within a team environment.
- Ability to effectively communicate both verbally and in writing.
- Current First Aid/CPR Level C; Suicide Intervention Certification (e.g. ASIST) is an asset.
- Computer literacy and effective working skills in MS Word, Excel, and email are required.
