Continuity Care Home Nurses is a licensed, insured and bonded state-of-the-art home health agency consisting of a multi-disciplinary team of health care professionals that are dedicated to the patient care philosophy of "continuity of care." Our personalized service, knowledgeable staff, and close monitoring of services provided create a warm, supportive, and caring environment for patients as well as family members. We provide Certified Nurse’s Aides & skilled nursing care (RNs, LVNs).... from four hours/day to 24 hours/day. Our second company Continuity Care Staffing Services, Inc. provides babysitting & caregivers to pediatric and geriatric patients.
We specialize in high tech home infusion visits & care to catastrophic patients (Ventilator Dependent, Spinal Cord Injury, Brain Injured) in addition to having PT, OT, ST, LCSW available. All cases are supervised by Registered Nurses who also case manage each patient. We can bill all private insurance plans, worker's compensation, long term care insurance, & accept all major credit cards. Our service area includes most of Southern CA.
