Job description

Position Summary:
Under the guidance of the Manager of Quality, is responsible for implementing and facilitating physician and practice performance improvement for the assigned region, including evaluation of current performance, planning improvement initiatives and ongoing measurement of physician performance and analysis of quality metrics and processes.
Essential Functions and Responsibilities:
1. Works with the Manager Quality and Snr QPSs to set priorities for MPP physician performance improvement for assigned region.
2. Assists practices in being nominated and designated as a Patient Centered Medical Home (PCMH) through evaluation of current performance and identifying improvement initiatives.
3. Works with the Manager Quality in developing and managing implementation plans to assist in practice transformation.
4. Responsible for educating and implementing process improvement tools (e.g. LEAN) in the physician practices for both current and new processes.
5. Conducts needs assessment at physician practices for readiness to implement technology (e-prescribing, Population Health Registry, EMR, etc.) and understand clinical workflows.
6. Following quality initiatives of contracted health plans, payers and governmental agencies, works with MPP Quality Team and practice to identify and support physician performance improvement strategies.
7. Provide education to MPP member physician and office staff on the self-management training program to improve patients' self-management of chronic conditions.
8. Other duties as assigned or when necessary to maintain efficient operations of the department and the Company as a whole.
Required:
  • Associate Degree in Health Care or a current Certifications in a recognized two-year health care program related to physician practice and clinical support operations.
  • Three (3) years of hands-on outpatient office or ambulatory care experience.
  • Valid state driver's license, reliable transportation and automobile insurance required and maintained throughout employment.

Preferred:
  • Bachelor's Degree in a medical or health related field
  • RN with a valid unrestrictive Michigan license.
  • Patient Centered Medical Home champion in a practice or plan setting.
