We offer in-home and community-based injection support, help patients navigate insurance and prior authorizations, and work closely with providers to overcome barriers to treatment. Job Overview:

In the Community Health Liaison (CHL) role, you will work under the supervision of the Community Health Liaison Manager. You will support AHCS Enhanced Clinical and Support Service Programs by working directly with behavioral health patients in clinic, parole, and home-based settings to improve medication adherence and overall health outcomes. You will also assist in coordinating care with clinical and pharmacy teams, providing patient support services, and identifying gaps in care to enhance treatment effectiveness. Success in this role will be demonstrated by improved patient engagement, adherence to treatment plans, and measurable optimization of health outcomes.

Responsibilities:

  • Conduct Enhanced Clinical Services to AHCS patients

  • Offer MTM and other clinical services to patients to optimize care

  • Conduct in person patient visits for enhanced services programs at partner organizations

  • Communicate and coordinate care with patients' healthcare provider and care team
  • Optimize outcomes in patient adherence and access to prescriptions
  • Attend meetings and work with other clinical team members
  • Complete required reporting requirements in a timely manner
  • Act as liaison between patient and pharmacy to ensure medication adherence
  • Assist pharmacy teams with insurance coverage lapses and benefit enrollment to ensure continuity of pharmacy care
  • Collaborate with AHCS sales team on potential new referral source, leads, and customers
  • Perform other tasks as required

Required Skills:

  • High school diploma or equivalent

  • Ability to work in a multi-cultural setting

  • Proficiency in Microsoft Office

  • Understanding of the behavioral health community

  • Effective communication skills verbally and in writing

  • Ability and willingness to provide emotional support, encouragement, and motivation to patients

Education and Experience:

  • Experience working in a community-based setting for 1 to 2 years

  • Successful completion of a Community Health Worker training program

  • Medical terminology and/or healthcare background

Working Conditions:

  • Full-time, typically requiring 40+ hours per week

  • Field work will require driving

  • Work is performed in a healthcare setting, including hospitals, clinics, and outpatient facilities, or visiting patient homes
