As a Community Support Specialist, you'll collaborate with clients, families, and other professionals to develop and implement comprehensive care plans tailored to individual needs and goals. Utilizing techniques like Motivational Interviewing, Positive Reinforcement, and Trauma-Informed Strategies, you'll facilitate skill-building and encourage growth. Your role will involve coordinating care, maintaining accurate records, and contributing to our organization's quality improvement initiatives.

Key Responsibilities:
  • Provide community-based mental health services, including assessment, diagnosis, treatment planning, and intervention implementation
  • Collaborate with clients, families, and stakeholders to develop and implement individualized treatment plans
  • Utilize evidence-based practices like Motivational Interviewing, Positive Reinforcement, Trauma-Informed Strategies, and CBT/DBT approaches
  • Coordinate care with service providers, including physicians, schools, and community organizations
  • Maintain accurate and timely records of client interactions, treatments, and progress
  • Participate in staff development activities and contribute to quality improvement initiatives
  • Foster relationships with key stakeholders involved in the client's care
