Job description

The Truancy Reduction & Assessment Center (TRAC) Case Manager provides assessment, referral, and caseย management services for middle and high school age youth demonstrating academic disengagement.ย Individual will work with students through individual meetings, working to track levels of engagementย through academic outcomes. Relationship-building strategies will be used to better understand underlyingย factors contributing to attendance concerns, thereby empowering youth through problem-solving toย support goal attainment.ย 

Schedule:ย 

  • Monday-Friday
    • 7:00AMย - 3:00PM or 8:00AM - 4:00PM

Essential Job Functionsย 

  • Develop trusting relationships with youth, families, school staff, Club staff, and communityย partners.
  • Collaborate with adults already involved with youth โ€“ such as teachers, case managers, counselors,ย and social workers.
  • Facilitate communication and promote problem solving between home and school to engageย families and promote school success.
  • Support youth in developing skills necessary for school success (i.e., time management, studyย habits, problem solving, goal setting, etc.).
  • Link youth to relevant Club and community programs including mentoring, tutoring, and group-based support and opportunities.ย 
  • Enter monthly and semester data into the TRAC database.
  • Engage in case consultation with supervisor, TRAC Team, Youth & Family Services Teams, andย others, as necessary.
  • Participate in school and community events related to truancy/attendance concern concerns.
  • Assist with BGCFV summer operations, as needed.
  • Utilize creative strategies to maintain contact with TRAC students as needed throughout theย calendar year.
  • Collaborate with internal teams and departments to align goals and strategies and to foster aย culture of cooperation and teamwork.
  • Engage in continuous learning and contribute to the effective implementation of continuousย improvement processes.ย 
  • Support the ethical use of data to make decisions in a timely and informed manner.
  • Represent the organization to the community in a way that enhances the organizationโ€™s profile,ย builds its reputation, supports its strategic priorities, and advances its mission.ย 
  • Contribute to operational goals and objectives that align with and advance the organization'sย strategic priorities.ย 
  • Support team development and performance, inclusive of but not limited to active listening, team-based learning, recognition, fun, and transparent communications.ย

  • Participate-in or conduct YFS Safety Checks, as scheduled.
  • Ensure current safety practices meet or exceed organizational requirements.
  • Support the development and implementation of new or amended policies, practices, andย processes that further mitigate and minimize safety risks.

The TRAC Case Manager position requires a majority combination of the following:ย 


Education and Experienceย 

  • Bachelorโ€™s degree in human services or related field.
  • Experience working with middle and high school-aged youth.
  • Experience working within a school system or youth agency.
