The Pediatric Program Assistant provides general support and assistance for hospice patient needs, the pediatric program office, patient support clinical interdisciplinary team, and general family communications.
Position Details
  • Work Location: The Dorian Family Pediatric Center at Community Hospice and Palliative Care, 4266 Sunbeam Road Jacksonville, FL. No work from home at this time.
  • Days/Hours: Monday - Friday, 8:00am - 5:00pm
  • Compensation: $17.00 - $19.00 hour, plus full healthcare benefits package including 23 days annual PTO

Job Description
Supports Pediatric Department Office Communications
โ€ข Answers inbound department phones and communicates staff messages accurately and timely to appropriate persons.
โ€ข Distributes patient paperwork and administrative documentation to appropriate managers
โ€ข Coordinates with the Durable Medical Equipment (DME) department relaying any appropriate patient equipment needs
โ€ข Maintains staff on-call schedule, including uploading monthly calendars onto the inner-department 'Connect' webpage and distributing weekly update emails
โ€ข Monitors re-certification process notifying clinical team members when re-certifications come due.
โ€ข Provides report on patient census population via the department EMR Suncoast software system
โ€ข Monitor and maintain Backline department communications for all needs
โ€ข Submits the CADD report weekly
โ€ข Corresponds with patient primary care physician office/ hospitals as needed.
โ€ข Relays communications with the Community Hospice Foundation Office for various events and needs
โ€ข Other duties as assigned.
Organizes and Maintains Patient Information
โ€ข Tracks team patient population census logging patient status changes, levels of care, admissions, deaths, discharges, and transfers
โ€ข Monitors the central Pediatrics 'PedsCare' email for department referral requests and patient family letters.
โ€ข Provides administrative EMR Forcura support, scanning and uploading designated documentation
โ€ข Prepares patient Interdisciplinary Group (IDG) agendas and corresponding spreadsheets with patient re-certifications into the EMR Suncoast system
โ€ข Coordinates and creates IDG meeting in Suncoast EMR system
โ€ข Prepare and distributes department handouts and documents for weekly team meetings
Event Supports
โ€ข Assists supervisor and pediatric staff with various department activities, planning events and team celebrations
โ€ข Orders materials, secures locations, interfaces with patient families, and helps direct department volunteers
โ€ข Coordinates vendor management and other participating departments.
โ€ข Tracks RSVP's for patient events and activities.
โ€ข Supports the Community PedsCare event as directed.
โ€ข Other duties as assigned.
Required Knowledge and Education
โ€ข Two (2) years of administrative experience including data entry and reporting.
โ€ข Experience in a health care environment preferred.
โ€ข Intermediate to advanced computer skills, word processing experience with the ability to type, file and do general clerical work. Experience with Excel spreadsheets and reports.
