Job description

Position Overview
Under the supervision of the Director of Care Management, Partnership in Care, with some latitude for professional judgement and independent decision making, the Community Outreach Supervisor will oversee the Community Outreach Team. This team will work collaboratively with staff conducting telephone care management to ensure that members experiencing challenges with care engagement are actively outreached in support of their return to care; they will also focus on supporting Medicaid and HARP members living with HIV with SNP enrollment and Medicaid recertification. Since some of the staff on the team will serve as facilitated enrollers, they will also supervise facilitated enrollment processes.

Scope of Role & Responsibilities

  • Provide care coordination, monitoring and follow-up for members with poor care engagement. Conduct face-to-face and telephonic case management support.
  • Provide daily supervision of the Community Outreach Team. Conduct in-field monitoring of staff on a weekly basis
  • Oversee the appropriate deployment of the Community Outreach Team to inpatient facilities, communitybased HIV clinics, and residential settings as needed to support members to return to care.
  • Supervise certified staff conducting SNP enrollment, overseeing the facilitated enrollment process.
  • Conduct trainings and workflow review with Community Outreach Team.
  • Monitor key performance indicators and outcomes for this team including, but not limited to, staff productivity, compliance with ACM model activities, return to care, viral load suppression, and HIV SNP enrollment, Medicaid recertification.
  • Implement logistical support technology and reports as needed to optimize personnel in the face of many needed in-person visits.
  • Ensure that the Community Outreach Team is implemented based on the outlined work plan. Monitor all the goals and objectives identified within the work plan.
  • Participate in the development, implementation and evaluation of quality improvement and performance improvement initiatives related to the Community Outreach Team including community-based outreach.
  • Attend community meetings/trainings, as needed.

Required Education, Training & Professional Experience

  • Bachelor’s Degree from an accredited college in Healthcare, Public Health, Public Administration, or related fields.
  • 5+ years of experience working in a managed care, hospital, or CBO setting including 1-3 years of supervisory experience.
  • Proficiency with Microsoft Office products is required.
  • Must be comfortable conducting home visits and outreach in diverse settings by themselves
  • Experience and knowledge of Managed Care plan operations of Health Services administration is preferred
  • Knowledge of concerns of persons living with HIV is preferred
  • Bilingual (English/Spanish) is preferred. Fluency in other languages is welcomed.
