Job Duties:

In this highly visible role, you will work directly with a care team to help patients overcome barriers to care, understand their health needs, and connect to resources that support their well-being. You will talk with patients, listen to their concerns, and help them navigate transportation, food, housing, chronic disease follow-up, and preventive care reminders. You will work closely with medical and behavioral health providers, reinforcing care plans and encouraging patient engagement. This role requires curiosity, compassion, good communication, and comfort using technology and documentation systems. If you enjoy problem-solving, learning, and building trust with people from different backgrounds, this work is both meaningful and impactful.

Requirements:

Required Qualifications:

  • Experience working with diverse populations.
  • Strong interpersonal and communication skills.
  • Ability to work both independently and within care teams.
  • Strong computer literacy and ability to learn EHR systems.
  • Strong organizational and time management skills.
  • Reliable transportation.
  • High school diploma or equivalent

Preferred Qualifications but Not Required:

  • Background in public health, allied health, or related field.
  • CHW Core Competency training or CHW Certification.
  • Experience in health or human service settings.
  • Experience with population health or quality improvement.
  • Experience with navigation, SDOH, or care coordination.
  • Experience with motivational interviewing or patient engagement.
