Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and childrenโ€™s and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Role

The Lead Housing Navigator/Lead Housing Case Manager plays a crucial role in supporting individuals with chronic health or mental health conditions, as well as those experiencing homelessness. This positionย is responsible forย developing and coordinating customized housing plans to ensure stable housing and connecting clients with essential community resources and services. The Lead is committed to enhancing the health and well-being of homeless individuals through supportive services, while upholding strict confidentiality and professionalism.ย In this leadership role, the Lead mustย demonstrateย strong leadership skills, including the ability to guide a team effectively in high-pressure situations. The Lead willย be responsible forย assistingย approximately 1-6ย team members and ensuring they receiveย necessaryย support.

The pay range for this position is $26.00 - $28.00ย per hour, depending on experience. Monday - Friday, 8:00AM to 5:30PM.

Responsibilities
Lead Duties:

  • ย  Shouldย be able to manage a caseload of 10 to 15 clients and express a willingness to accommodateย additionalย cases if requested.ย 

  • The Lead is also expected toย identifyย and implement solutions to overcome barriers, address conflicts, recommend workflow optimizations, and troubleshoot any system inefficiencies that may arise.ย 

  • The Lead will collaborate with and train team members by providing onboarding, developing training materials, and offering guidance through shadowing opportunities.ย 

  • Work with Manager inย determiningย strengths and weaknesses of theย Cal AIMย policies, structures, and program.ย ย 

  • Assistย in training and providing resources to Housing Navigator/Housing Caseย Managers.ย ย 

  • Train Housing Navigators/ Housing Caseย Managersย and encourageย teamwork, growth, efficiency, and effectiveness.ย ย 

  • Attendย Interdisciplinary Care Teamย meetingsย ย 

  • Collaborate withย Managerย to discuss team operations and program updates.ย ย 

  • Collaborate withย Managerย inย providing solutions to barriers in staff and client care during team meetings.ย 


Housing Duties:

  • Develop individualizedย housingย plans with measurable goals andย objectivesย andย assistย participants in achieving clients' desired outcomes.ย ย 

  • Jurisdictionย isย determinedย by the relevant county associated with each role, such as Orange County, Inland Empire, or Los Angeles.ย 

  • Must be willing to travel to various locations and engage with clients in the field.ย 

  • Collaborate withย Managerย to discuss updates about staff and/or program.ย ย 

  • Assist in training and provide resources toย continuingย increaseย knowledge.ย 

  • Exemplify leadership by displaying IFโ€™s Guiding Principles.ย ย 

  • Leadย meetingย and occasionally act as theย supervisoryย figure, ifย manager is not present.ย ย 

  • Demonstrate leadership by guiding and collaborating with CSS HN/ HCMย ย

  • Complete training and development opportunities asย determinedย appropriate by the supervisor.ย ย 

  • Coordinate, prepare, andย maintainย necessary charting and documentation promptly and comprehensivelyย in accordance withย the key performance indicators (KPIs)..ย 

  • Attend relevant subcommittee and implementation meetingsย ย 

  • Seekย out innovative housing solutions for client housingย ย 

  • Conduct housing inspections to meetย Housing and Urbanย Development, and Housingย Habitabilityย (if applicable)ย 

  • Work with Housing Navigators/ Housingย Caseย Managersย to develop housing goal plans and make referrals to appropriate resourcesย if applicable.ย ย ย 

  • Assistย in the training of new employees and oversee their caseloads by fostering the professional development, efficiency, and effectiveness of staff.ย 

  • Attaining services such as housing, mental health care,ย employment, financialย assistance, legal advocacy, etc.ย (if applicable)ย ย 

  • Develop relationships with representatives within Illumination and in other agencies to support individualsย ย 

  • Act as an advocate for clients to ensure the maximum benefits and resources are obtained andย maintainedย toย ensure client's ability to becomeย self-sufficientย ย 

  • Network and collaborate with housing providers, landlords and property managers to develop housing stock for program. Includes temporary and permanent housing alternatives.ย

Preferred Experience/Minimum Qualifications

Required:

  • 2 Years of Housing Navigation or Case Management Experience, or similar fieldย ย 

  • Understanding of housing rental market and property managementย 

  • Understanding ofย Homelessness and at risk of Homelessness according to HUD requirements.ย 

  • Proficiencyย inย Microsoft (Outlook, Teams,ย SharePointย andย Calendar).ย ย 

  • Customer and client relations skillsย 

  • High School Diploma or equivalent.ย ย 

  • Possession of a personal vehicle isย required.ย 

  • Valid CA Driverโ€™s License and eligibility for company vehicle insurance.ย ย 

Preferred:ย ย 

  • Bilingual in English and Spanish.ย ย 

  • Bachelor's degree or at least one year of case management experience or similarย positionย  Homelessย services, case management, mental health, permanent supportive housing experienceย
