This position is a remote role where you will collaborate with a diverse team of professionals dedicated to providing care in a Hospice Center setting. You will have the flexibility of a remote workspace while still actively engaging with clients through virtual appointments, phone consultations, and community outreach initiatives. In your role as a Licensed Clinical Social Worker, you will:

• Establish rapport and trust with clients through empathetic engagement and active listening.
• Conduct comprehensive assessments and diagnostics of clients presenting with symptoms of major mental illness or severe emotional disorders.
• Craft and implement personalized treatment plans that encompass a wide range of prevention, intervention, and recovery strategies.
• Utilize evidence-based practices to ensure the efficacy of your therapeutic interventions.
• Provide crisis intervention services and support to clients in acute distress, ensuring their safety and well-being.
• Engage in multidisciplinary collaboration to foster a holistic approach to client care, preventing the fragmentation of services.
• Maintain meticulous electronic health records, ensuring compliance with legal and ethical standards while facilitating timely reporting and data submission.
• Participate actively in quality improvement initiatives, contributing to ongoing assessment and enhancement of clinical practices.

Your efforts will primarily focus on serving underprivileged communities, particularly in the context of mental health challenges experienced by individuals in Hospice care. You will specialize in providing support to clients dealing with grief, loss, and associated psychological distress, fostering resilience and promoting coping mechanisms to enhance their quality of life during difficult times.

The ideal candidate will possess:

• A Bachelor’s Degree in Social Work or a related field, with state licensure as a Licensed Clinical Social Worker (LCSW).
• At least 2 years of relevant clinical experience, emphasizing assessment, diagnosis, and treatment of mental health issues among vulnerable populations.
• Demonstrated ability to navigate complex social systems and advocate for clients effectively.
• Outstanding verbal and written communication skills for effective collaboration with colleagues and interacting with clients.
• Strong organizational and planning skills to manage a diverse caseload efficiently.

While not mandatory, preference will be given to candidates with the following:

• A Master’s degree in Social Work (MSW) or a related field.
• Bilingual proficiency in English and Spanish, enabling better communication with a broader client base.
