Position: Clinical Admissions Coordinator - Full Time
We are seeking a clinically experienced professional to join our team as a Clinical Admissions Coordinator, playing a vital role in evaluating patient needs, guiding care transitions, and supporting appropriate admissions into our center.
This position is ideal for a nurse or clinically trained individual looking to step into a more collaborative, non-bedside role while continuing to make a meaningful impact on patient outcomes.
Qualifications
  • Licensed Nurse (LPN or RN) preferred, or equivalent clinical experience
  • Bachelor's degree preferred from an accredited college or university
  • Strong understanding of medical case management and care transitions
  • Knowledge of geriatric patient populations and post-acute care settings
  • Working knowledge of funding resources including Medicare (A, B, C, D), Medicaid, Managed Care, and private insurance
  • Insurance authorization experience preferred
  • Ability to interpret clinical information and medical terminology
  • Strong communication and relationship-building skills
  • Marketing or referral development experience is a plus
Key Responsibilities
  • Review and coordinate incoming referrals, using clinical judgment to assess appropriateness for admission
  • Serve as a liaison between hospitals, providers, patients, families, and internal care teams
  • Conduct thorough needs assessments and help guide patients and families through the admissions process
  • Collaborate with interdisciplinary teams to support safe and appropriate care transitions
  • Communicate funding options and assist with navigating insurance and authorization processes

  • Represent the center professionally with referral sources and community partners
