Griswold Home Care is seeking a compassionate, organized, and service-driven Client Care Coordinator to join our growing team. This role is essential in ensuring our clients receive the highest quality of care and that our caregivers feel supported and set up for success.
About the Role
The Client Care Coordinator is responsible for managing client relationships, overseeing service schedules, supporting caregivers, and ensuring seamless communication between clients, families, and our office team. You will be a key part of delivering the exceptional care Griswold is known for.
Responsibilities
  • Serve as the primary point of contact for new and existing clients
  • Conduct client intakes, assessments, and ongoing check-ins
  • Coordinate caregiver schedules and match caregivers to clients based on needs and skillsets
  • Support caregivers with updates, training reminders, and problem-solving
  • Maintain accurate client records and documentation
  • Assist in resolving scheduling challenges or client concerns in a timely, professional manner
  • Collaborate with the office team to ensure smooth operations and excellent service
