Care Coordinator & Lead Caregiver

Preferred Care at Home of Apex, Garner &ย Fuquay-Varinaa

Are you an experienced caregiver ready to take the next step into coordination and leadership?

Preferred Care at Home of Apex, Garner, and Fuquay-Varina is seeking a dependable, organized care professional to manage caregiver schedules, support new client starts, and provide direct care when needed.

This is a hands-on role combining scheduling, problem-solving, caregiver support, client communication, and caregiving.

Key Responsibilities
  • Manage client and caregiver schedules in AxisCare
  • Match caregivers with clients based on skills, availability, location, and personality
  • Respond to call-outs, late arrivals, missed clock-ins, and open shifts
  • Continue working an open shift until a safe solution is found
  • Personally cover client shifts when necessary
  • Coordinate caregivers and details for new client starts
  • Attend or work first shifts when helpful
  • Follow up with clients, families, and caregivers
  • Support and coach caregivers in the field
  • Review attendance, clock-in, and documentation issues
  • Communicate concerns and escalate safety or clinical matters appropriately
  • Provide companionship and assistance with personal care, mobility, meals, transportation, and other approved services
Qualifications
  • Previous hands-on caregiving experience
  • Strong organization, communication, and follow-through
  • Ability to remain calm and solve problems when schedules change
  • Comfortable providing personal care and covering client shifts
  • Comfortable using scheduling and communication technology
  • Reliable personal transportation and a valid driverโ€™s license are required
  • Ability to travel throughout Apex, Holly Springs, Fuquay-Varina, Garner, and surrounding South Wake County communities
  • Ability to meet all agency background, health, training, and competency requirements

Previous scheduling, staffing, home care, assisted living, or supervisory experience is preferred. CNA, PCA, medication aide, medical assistant, CPR, or First Aid credentials are also helpful.

Schedule, Location, and Compensation

This is a full-time, hourly position combining office work, remote administrative work, and visits to client homes.

After training, some scheduling and administrative responsibilities may be completed remotely on designated days. Hybrid flexibility will depend on the care schedule and operational needs. The employee must remain available to report to the Apex office or a clientโ€™s home when needed.

The normal schedule will primarily involve weekday daytime hours, but reasonable flexibility is required for early mornings, evenings, weekends, new client starts, call-outs, and urgent coverage needs.

Because travel throughout South Wake County is required for success in this role, candidates must have reliable personal transportation and a valid driverโ€™s license.

Pay: $16โ€“$18 per hour, depending on experience, qualifications, and responsibilities.
