Care Coordinator & Lead Caregiver

Preferred Care at Home of Apex, Garner & Fuquay-Varinaa

Are you an experienced caregiver ready for your next step?

Preferred Care at Home of Apex, Garner & Fuquay-Varina is growing, and we’re looking for a unique person to grow with us.

This is not a traditional caregiver position.

We’re looking for an experienced caregiver who is also organized, comfortable with technology, an excellent communicator, and great at solving problems. You’ll spend part of your time helping coordinate our caregivers and client schedules from our Apex, NC office and part of your time serving as a Lead Caregiver, providing care when additional or short-notice coverage is needed.

This is an opportunity for someone who loves caregiving but is ready to take on more responsibility and develop experience in the operational side of home care.

What You’ll Do

Care Coordination & Scheduling

You’ll help make sure our clients have the dependable care we’ve promised them.

Responsibilities may include:

    • Coordinating and maintaining client and caregiver schedules
    • Maintaining accurate caregiver availability
    • Matching caregivers with clients based on availability, location, skills and client needs
    • Communicating schedules and schedule changes with caregivers
    • Helping fill open shifts and responding to caregiver call-outs
    • Proactively reviewing upcoming schedules and identifying potential coverage issues
    • Working closely with our Caregiver Recruiter to identify current and upcoming staffing needs
    • Helping newly hired caregivers get connected with appropriate clients and shifts
    • Checking in with caregivers and helping resolve routine scheduling and field issues
    • Communicating with clients and families regarding scheduling when appropriate
    • Helping maintain continuity of care and strong caregiver/client matches
    • Assisting with other day-to-day office and operational needs

Lead Caregiver & Backup Coverage

You’ll also remain active in the field.

When a caregiver becomes unexpectedly unavailable or we have another short-term coverage need, you may step in personally to ensure our client receives the care they are expecting.

Depending on the client, care may include:

    • Companionship and supervision
    • Bathing, dressing, grooming and other Activities of Daily Living (ADLs)
    • Mobility and transfer assistance
    • Alzheimer’s and dementia care
    • Meal preparation
    • Light housekeeping
    • Transportation, errands and appointments
    • Following individualized care plans
    • Observing and reporting changes in a client’s condition
    • Providing compassionate support that promotes independence, safety and dignity

Who We’re Looking For

This position requires a combination that doesn't fit just anyone — a great caregiver who is also a great organizer and problem-solver.

You may be a great fit if you:

    • Have proven professional caregiving experience
    • Are extremely dependable and take ownership when something needs to get done
    • Are organized and able to manage multiple priorities
    • Are comfortable working on a computer and learning scheduling software
    • Communicate professionally by phone, text and email
    • Stay calm when plans change unexpectedly
    • Enjoy solving problems rather than waiting for someone else to solve them
    • Can work independently and make good decisions
    • Are comfortable communicating with caregivers, clients and families
    • Are willing to step into client care when coverage is needed
    • Have a compassionate and patient approach to senior care
    • Want an opportunity to grow beyond a traditional caregiving position

CNA or PCA experience is a plus. More importantly, we’re looking for the right combination of caregiving experience, reliability, organization, communication skills and leadership potential.

Schedule

This position will initially be approximately 30+ hours per week, primarily during normal weekday daytime hours, hours can grow with the business  

Because an important part of this role is helping our agency respond when unexpected staffing needs arise, weekday flexibility and the ability to occasionally provide client care on relatively short notice are important.

This is not a 24/7 on-call position.
