Position Summary

The Medical Records Clerk is responsible for maintaining, organizing, and processing patient medical records in accordance with organizational policies, confidentiality requirements, and applicable healthcare regulations. This position supports accurate and timely management of medical documentation and assists clinical and administrative staff with record-related requests.

Essential Duties and Responsibilities
  • Maintain and organize electronic and paper medical records.
  • Scan, upload, file, and index medical documentation into the appropriate patient record.
  • Verify that medical records are complete, accurate, and properly identified.
  • Retrieve and distribute medical records and documentation as authorized.
  • Process requests for medical records in accordance with organizational policies and applicable privacy requirements.
  • Assist with release-of-information requests and ensure appropriate authorization is obtained.
  • Perform quality checks to identify missing, incomplete, or incorrectly filed documentation.
  • Communicate with clinical, administrative, and external parties regarding medical record requests.
  • Maintain confidentiality of protected health information (PHI) at all times.
  • Follow HIPAA, privacy, security, and records-retention requirements.
  • Assist with audits and record reviews as requested.
  • Maintain an organized and secure medical records area.
  • Perform other clerical and administrative duties as assigned.
Qualifications

Required:

  • High school diploma or equivalent.
  • Must be 21 or older
  • Previous clerical, medical records, healthcare administrative, or related experience preferred.
  • Basic computer and data-entry skills.

Preferred:

  • Experience with an electronic health record (EHR) system.
  • Knowledge of HIPAA and medical-records practices.
  • Previous experience in a healthcare or behavioral-health setting.
