Sage Group HealthCare Staffing is seeking Hospice RN Case Manager for a travel/contract position in Blue Ash, OH. Accepting both local and traveler candidates.
Position Details:
Position Name: Hospice Registered Nurse Case Manager
Location: Blue Ash, Ohio
Specialty: Hospice
Start Date: ASAP
Employment Type: Full-time, Travel, Contract, Temporary
Contract Length: 8-week contract with extensions.
Pay Rate: Range $2080 to $2120 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Hospice in a home Setting
Shift available: Days
Shift timings: 8-hour Days, 8.00 am to 5.00 pm
Schedule: 40 hours/week guaranteed. 5 X 8-hrs, 5 shifts per week, Mon-Fri. On-call every other weekend from Friday, 5 pm through Monday, 8 am
Position Requirements:
  • Registered Nurse: 2years - Required
  • Recent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years - Required
  • Home Care Home Base(HCHB) charting experience - Required
  • Must have validDriver's License and Personal Auto Insurance
  • Must be Covid Vaccinatedor provide medical or religious exemption
Education, License, and Certification Requirements:
  • ASN or an equivalent- Required
  • BSN or an equivalent- Highly preferred
  • RN License for the Stateof Ohio or Compact - Required
  • BLS Certificationfrom AHA or American Red Cross - Required
Job Description:
  • Provide and coordinate care for assigned Hospice patients and manage assigned caseload
  • Develop and implement nursing care plans for patients as assigned.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians' orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients' status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting
