Job description

POSITION PURPOSE AND SUMMARY
The Social Worker reports to the Director of Case Management. Under the direction of Lead Social Worker, the Social Worker assists with implementation of patient continuity of care by arranging discharge plans. In addition, the Social Worker provides supportive counseling to the patient, family and significant other/support person.
EDUCATION/EXPERIENCE:
  • The Social Worker shall have completed a Bachelor's degree in Social Work with a minimum of two years of experience in social work practice, preferably in a medical setting and be eligible to take the Indiana Health Professions Bureau Licensing Examination.
  • The Licensed Social Worker shall have completed a Bachelor's degree in Social Work with a minimum of two years of experience in social work practice, preferably in a medical setting and passed the Indiana Health Professions Bureau Licensing Examination.

Note: Prior to 1992 a Bachelor's degree in a field other than Social Work and appropriate professional work experience was acceptable to meet licensure requirements of the Indiana Health Professions Bureau. In addition, the exam may have also been waived for those individuals according to the grandfathering clause in place with the Indiana Health Professions Bureau at the time of their licensure
