Child Services Case Manager
Position Overview
The Case Manager supports children in conservatorship by coordinating services, managing case plans, and working toward permanency outcomes such as reunification, kinship placement, or adoption. This role collaborates closely with families, caregivers, and partner agencies to help ensure children are placed in safe, stable, and supportive environments.
Work Location: Field-Based Position
Key Responsibilities
- Manage a caseload of children placed in care and support permanency planning efforts.
- Assess safety, family needs, and placement options for children and caregivers.
- Develop and implement service and permanency plans that support reunification, kinship care, or adoption.
- Conduct home visits, assessments, and meetings with families and stakeholders.
- Maintain accurate case documentation, reports, and records in compliance with regulations.
- Coordinate services with internal teams, courts, and community agencies.
- Provide guidance and support to families, caregivers, and children throughout the case process.
- Respond to crisis situations and provide on-call support when required.
- Participate in staff meetings, trainings, and program activities.
Required Qualifications
- Associate’s degree in Child Welfare or a related field or 60 college credit hours and two years of relevant experience
- Relevant experience may substitute for education.
