Your future role at a glance

Location: Local and national travel options are available dependent on need.

Facility: Multiple Ascension acute care hospitals

Department: Associate Travel Program - Case Management

Schedule: Day shift available | 40 hours per week

Gross Rate up to $2,620 per week

How you'll make an impact in this role
  • Develop and integrate patient/family-centered care plans.
  • Continuously assess care via medical record review, securing resources, and removing care barriers.
  • Coordinate with supervisors and providers for effective care implementation.
  • Manage documentation, verify service appropriateness, and advise on post-discharge treatment.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Registered Nurse obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
