Job description

Agape of Appleton, a non-profit human services agency providing residential support services to individuals with differing abilities, is seeking a full-time Residential Case Manager to lead our Agape AFH program. The Residential Case Manager provides case management, program management, staff supervision, and direct care support to individuals.
**Now hosting weekly open interviews on Thursdays from 9am to 3pm CT.**

Key Responsibilities for the Residential Case Manager:
  • Provide inter-intra agency case management
  • Develop, update and implement Individual Support Plans (ISPs) and ensure individuals are receiving appropriate authorized care and services per contractual agreements
  • Maximize the social functioning of each individual by promoting community integration
  • Provide support and advocacy in an effort to safeguard the human and civil rights of each individual while fostering their dignity and self-worth
  • Provide guidance to direct care staff and support to individuals when challenging behaviors occur
  • Provide crisis response as needed
  • Provide leadership and guidance to direct care staff, including training, coaching, mentoring, and performance management
  • Provide direct care support to individuals based on program needs and staffing levels
  • Program currently has 3 participants but can support 4.

Qualifications for the Residential Case Manager:
  • Associate degree from an accredited college in a health-care related field or Bachelor's Degree from an accredited college and experience working in a health-care related field
  • At least 2 years previous management experience is preferred
  • Experience working with individuals in a group home setting is a plus
  • Must be at least 21 years of age
  • Maintain a valid driver license with excellent driving record
  • Be mentally and physically capable of assisting participants based on their needs.
  • Be able to use sound judgment in carrying out written and oral direction
  • Ability to generalize and apply information in a variety of circumstances to maintain consistent support to individuals, especially during an emergency.
  • Ability to be flexible and adaptable in hours of work, environments, and situations.
  • Ability to encourage and promote personal growth and development in others
  • Sufficient experience/education to meet the position requirements or the ability to acquire and maintain such requirements through training per agency and regulatory standards
  • Visa sponsorship is not offered for this position. Candidates must be legally authorized to work in the United States.
