Position: W2 Case Manager/FEP

Location: Oshkosh, WI or Wautoma, WI

In this role, you will help inspire hope for our program clients and work with them to build upon their strengths toward reaching their dreams. Responsibilities include eligibility determination, benefit issuance, case management, and community collaboration. Additionally, the W-2 Case Manager/FEP promotes the Transition to Success (TTS) model and supports Forward Service Corporation (FSC)’s mission through effective communication and exceptional client support.

Position Overview- Key Responsibilities (Full details on attached job description):

  • Manages individual caseloads using a family-based, holistic approach.
  • Determines eligibility for W-2 and related programs in accordance with program policies.
  • Coordinates with community partners through the CARE Network to connect clients with services, including education, training, assessment, volunteerism, and employment.
