Private Pay (Non-Medicare/Non-Medicaid) Home Health and Personal Assistance Service Agency is looking for a full-time RN Case Manager. Agency is young but Administrator is very experienced in providing private nursing and personal assistance care. Administrator is an industry veteran and looking for just the right person to join the team!

Be a part of an enthusiastic team and help grow this private pay agency! The RN Case Manager is a registered professional nurse who ensures quality of patient care in the home or wherever the client calls home. Primary function is to manage a case load of private pay clients, perform initial assessments, meet with new clients to coordinate care and develop the care plan or plan of care and to manage the care team that is delivering service to the client in collaboration with the internal team. Coordinate care with the interdisciplinary team client/family and referring party. Bi-lingual in Spanish a PLUS ! Home Health experience is a huge plus!

Graduate of an accredited School of Nursing and is a Registered Nurse licensed in Texas or in accordance with the Texas Board of Nursing rule for the Nurse Licensure Compact.

Bi-lingual Spanish is a PLUS ! Home Health experience is a HUGE plus!

Minimum of 2 years as a Registered Nurse. Management experience preferred.

Must have excellent nursing skills, acute care experience and/or rehab experience a plus.

Must have excellent customer service skills and an innate understanding of superior service and high standards for clinical excellence.

Must be a positive team player with strong inter-personal skills
