Job description

Amazing Care Home Health is seeking a motivated, compassionate, and experienced Registered Nurse (RN) Case Manager to support the continued growth of our Pediatric Private Duty Nursing (PDN) services throughout Central Texas.

This is a unique opportunity for a nurse who enjoys both clinical leadership and direct patient care. The RN Case Manager will oversee pediatric cases across a large service area while partnering with families, clinicians, referral sources, and internal leadership to ensure exceptional patient outcomes and continued market growth.

The assigned territory spans North to South from Hillsboro to Kyle and East to West from Bryan to Waco, providing an exciting opportunity to make a meaningful impact throughout the region.

Schedule:

  • Full-Time
  • Monday–Friday
  • Participation in On-Call Rotation
  • Combination of office and field responsibilities

Field Expectations:

This role includes both office and field responsibilities to ensure consistent clinical oversight throughout the assigned service area.

Field expectations include:

  • 1–3 scheduled visits per week: Minimum of 16 hours dedicated to field work.
  • 4–5 scheduled visits per week: Minimum of 8 hours dedicated to field work.

Key Responsibilities:

  • Manage a caseload of pediatric Private Duty Nursing (PDN) patients throughout the assigned territory.
  • Coordinate patient care while serving as the primary clinical resource for families and field clinicians.
  • Conduct supervisory visits, patient assessments, and skilled nursing visits as needed.
  • Monitor patient progress and collaborate with physicians, families, and interdisciplinary team members.
  • Complete Quality Assurance (QA) reviews to ensure documentation accuracy and regulatory compliance.
  • Maintain complete and accurate electronic medical records.
  • Conduct interviews for prospective nursing staff and support the recruitment and onboarding process.
  • Partner with scheduling and talent acquisition teams to ensure appropriate clinician placement.
  • Support referral development and market growth by building relationships with physicians, hospitals, case managers, and community partners.
  • Represent Amazing Care at community events, networking opportunities, and career fairs.
  • Collaborate with the Director of Nursing and leadership team to support operational initiatives and quality improvement efforts.

Requirements

  • Current Texas Registered Nurse (RN) license in good standing.
  • Bilingual (Spanish) Preferred
  • Minimum of one year of nursing experience required.
  • Pediatric home health or Private Duty Nursing experience preferred.
  • Previous case management or leadership experience preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and organizational abilities.
  • Proficient with electronic medical record (EMR) systems.
  • Ability to work independently while managing a large geographic territory.
  • Valid Texas driver's license, reliable transportation, and current auto insurance.
