About the Role

The Case Manager will develop, implement, and evaluate individualized patient care plans. The Case Manager will advocate for patient welfare, and serve as a liaison between patients, their families, and healthcare providers. 

POSITION’S ESSENTIAL RESPONSIBILITIES: 

  • Documenting clients’ case management plans and on-going activities. 
  • Ongoing review of precertification requests of medical necessity. 
  • Monitor the activities of clinical and non-clinical staff. 
  • Identifying clients’ insurance coverage or other sources of payment for services. 
  • Identifying and addressing client risk factors and/or obstacles to care. 
  • Identifying client needs, current services, and available resources, then connecting the client to services and resources to meet established goals. 
  • Communicating the care preferences of clients, serving as their advocate, and verifying those interventions meet the client’s needs and goals of treatment. 
  • Screening clients and/or population for healthcare needs. 
  • Developing a client-focused case management plan. 
  • Educating the client/family/caregiver about the case management process and evaluating their understanding of the process. 
  • Floating among various clinical services, where qualified and competent. 
  • All other duties as assigned. 

EDUCATION AND EXPERIENCE REQUIREMENTS: 

  • Graduate of an accredited nursing program required
  • Minimum of TWO years’ experience required in Case Management. 
  • Medical terminology knowledge. 
  • Experience with MDS completion, reimbursement, clinical resource utilization and/or case management is highly desirable.
