Job description

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.

The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.

This role is different because Case Manager, RNs at WelbeHealth:

On the day-to-day, you will:

  • Conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop participant care plans
  • Deliver and document nursing interventions as agreed upon in participants' care plans, including but not limited to maintaining a safe environment, promptly responding to physician orders, and correctly administering medications
  • Collaborate with the WelbeHealth PACE Physician and other members of the IDT in coordinating participant care needs, as well as communicate with participants and their families to revise care plans
  • Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies

Job requirements include:

  • Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
  • Unencumbered California RN License
  • Current Basic Life Support (BLS)/First Aide Certification required
  • Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
