CANDIDATE PROFILE:

Latino Network is seeking a bilingual, mission-driven Case Manager passionate about fostering the mental health and wellness of Latine communities by joining Zenit Centro de Serenidad, Latino Network's culturally specific mental health clinic. They bring a deep commitment to equity and have experience working directly with youth and families, particularly in navigating access to critical social services such as rental assistance, utility support, food security, and healthcare. Highly self-motivated and well-organized, the candidate thrives in a dynamic, collaborative team environment and demonstrates strong interpersonal skills that foster trust and meaningful relationships with community members. They are comfortable providing front desk support such as scheduling, engaging in outreach,  case management, and advocacy work, and are eager to support the vision and mission of Latino Network's Mental Health Program through culturally specific care and community engagement.


POSITION DESCRIPTION:

Ability to meet the requirements for a Qualified Mental Health Associate as defined by the Oregon Administrative Rules and Mental Health and Addiction Certification Board of Oregon (MHACBO). The Case Manager position also requires Community Health Worker (CHW) certification or the ability to obtain certification within six months of hire. Community Health Workers are trusted frontline public health professionals who serve as bridges between healthcare systems and the communities they serve. Through relationship-building, advocacy, and culturally responsive support, CHWs improve access to services and strengthen the quality of care delivery.

The Case Manager will primarily work in person at Zenit Centro de Serenidad and may also support community outreach efforts at locations including safety-net clinics, shelters, libraries, DHS offices, health fairs, schools, home visits, and community events. This role serves as one of the first points of contact for individuals seeking mental health services and plays a key role in reducing barriers to care.

This position requires a bilingual, flexible, and self-directed professional with strong organizational, communication, and problem-solving skills. The Case Manager will provide front desk coverage, support clients in navigating healthcare and social service systems, assist with appointment coordination and follow-up, provide health education and skill-building support, refer clients to existing Latino Network programs, and maintain accurate documentation of services provided.

This role offers an opportunity to make a meaningful impact in the Latine community while gaining valuable experience in culturally responsive mental health care and community engagement.


RESPONSIBILITIES:

  • Support recruitment, scheduling, outreach, and engagement strategies using culturally and developmentally appropriate communication.
  • Serve as a welcoming first point of contact for individuals seeking services at Zenit Centro de Serenidad 
  • Assist with front-desk duties and client reception.
  • Conducts in depth intakes thorough a culturally specific, trauma informed lens.
  • Coordinate closely with the Clinical Supervisor and Clinic Coordinator to support smooth clinic operations.
  • Maintain accurate and timely documentation of participant interactions and services in Reliatrax (and other required systems)
  • Responsible for collecting sensitive demographic information for ROADS submissions.
  • Works closely with clinicians on case coordination to support with client care.
  • Support clients in accessing community resources including housing, food assistance, healthcare, transportation, and substance use services.
  • Carry a caseload of up to 50 clients.
  • Provide skill-building support to clients and families.
  • Utilize trauma-informed and culturally responsive practices in all client interactions.
  • Support culturally relevant mental health education, workshops, and community outreach initiatives.
  • Collaborate with internal staff and external partners to strengthen community support systems and improve access to care.
  • Facilitate support groups and skill-building circles for clients.
  • Assist clients with appointment reminders, forms, referrals, and navigation of health and social service systems.
  • Advocate for clients and empower them to actively participate in their wellness goals.

Requirements

REQUIRED SKILLS/KNOWLEDGE/ABILITIES

  • Certified as a Community Health Worker or able to complete training approved by the Oregon Health Authority and become certified within 6 months of hire.
  • A combined 3 years of education and relevant work experience in the healthcare system, community health, outreach, education, social services, community engagement  and social service resource navigation is required.
  • Ability to meet the requirements for a Qualified Mental Health Associate as defined by the Oregon Administrative Rules and Mental Health and Addiction Certification Board of Oregon (MHACBO). 
  • Bilingual in Spanish and English required.
  • Strong interpersonal communication, ability to establish trust, build rapport, and communicate effectively with individuals and groups from diverse cultural and socioeconomic backgrounds.
  • A deep understanding of and respect for the cultural, linguistic, and historical contexts of Latine communities.
  • Ability to manage multiple responsibilities, maintain accurate documentation, and communicate effectively across diverse audiences.
  • A passion for serving Latine communities and empowering individuals to achieve improved health and well-being.
  • Strong attention to detail in documenting client interactions, maintaining accurate records, and preparing reports.
  • Experience with receptionist duties, medical scheduling, outreach, advocacy, or program coordination is preferred.
  • Proficiency in Reliatrax electronic health record systems preferred.
  • Familiarity with common mental health conditions, stigma reduction strategies, and crisis intervention techniques.
  • Understanding of social determinants of health, health disparities, and approaches to address them.
  • Ability to manage multiple tasks, prioritize work effectively, and meet deadlines.
  • Proficiency in connecting individuals to resources, advocating for their needs, and guiding them through mental healthcare and social service systems.
  • Reliable transportation required. Automobile, driver's license, and automobile liability insurance preferred.
  • Must be eligible to be an insured driver under Latino Network's Commercial Auto Liability policy.
