Job description

Care Manager – Acute Medical

This is a 13-week contract position.

Day shift, 5x8 hours, from 08:00 AM to 04:30 PM.

The Care Manager will float to various facilities, primarily the Laguna Facility, managing care coordination for acute medical patients. The role requires extensive experience in discharge planning, care assessments, and coordinating with interdisciplinary teams to achieve optimal patient outcomes.

Responsibilities:

  • Perform patient assessments and coordinate care ensuring comprehensive treatment plans.
  • Facilitate family meetings and develop shared treatment goals.
  • Arrange durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport as needed.
  • Ensure compliance with CMS and Joint Commission standards.
  • Support diverse patient populations, including those experiencing homelessness or mental health crises.

Required Experience / Certifications / Licensure:

  • Minimum of 2 years of acute medical care management experience.
  • Experience with care coordination including discharge planning.
  • CA state license required; pending licenses accepted if fully licensed by start date.
  • Proficient in Epic charting system.
  • Must be available for weekend rotations and holiday shifts.
  • Travel experience required; refusal to float may result in termination of assignment.
