Inpatient Case Manager - Travel Contract
Job Description
We are seeking a dedicated Inpatient Case Manager for a travel contract opportunity. This position involves providing care coordination for neuro and cardiac patients in a comprehensive stroke center. The ideal candidate will have experience in inpatient acute care and case management, as well as familiarity with the Epic charting system.
Responsibilities
  • Coordinate care for neuro and cardiac patients.
  • Engage in discharge planning and needs assessment.
  • Order durable medical equipment (DME) as needed.
  • Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
  • Provide support and guidance to patients and their families.
Required Experience / Certifications / Licensure
  • Minimum of 1 year of inpatient/acute care experience.
  • Experience in case management is required.
  • Active BLS certification is required.
  • Previous experience with the Epic charting system is essential.
  • Travelers must have been separated from any previous assignments for at least 3 months.
Schedule
This is a day shift position with hours from 08:00 to 16:30. Weekend coverage is required, with a minimum of 2 weekend days per month.
