PRIMARY FUNCTION:

Case manager will assist women in Pregnant/Parenting Women’s Residential Treatment and childcare program with transportation and needed services in the community during their treatment stay. This position assists patients needing assistance with obtaining ID, food handler’s permits, community services while in residence, etc. Case Manager may also need to help patients acquire essential life skills for managing a household, time management, budgeting, cooking, housekeeping, accessing childcare, computer and technology skills, restoring credit, applying for permanent housing, or identifying appropriate community support services.

CLASSIFICATION:

Full-time hourly (non-exempt), requiring some evening and/or weekend hours.

PRIMARY RESPONSIBILITIES:

The PPW Generalist Case Manager assists residents to carry out identified external service planning, coordination, advocacy, and monitoring of support services as identified in patients’ residential individual service plans, pre-discharge planning, and continuing care activities after discharge.

• Develops, organizes, and maintains a comprehensive resource binder with up-to-date contacts across systems and agencies.
• Identifies and assists in removing barriers to accessing services as part of discharge planning (i.e., issues of food, housing, or transportation) and supports the clinical plan of care as developed by the clinical team.
• Coordinates care, including arranging or providing transportation, with doctors, outpatient SUD and mental health providers, housing programs, dental, medical and pediatric health provider appointments, employment and vocational programs, public entitlement applications, spiritual or religious programs designated by clients, and community self-help support groups, e.g. NA or AA.
• Assists clients with building life skills as outlined in the individual service plan, such as cooking, cleaning, using public transportation, banking, budgeting, scheduling appointments, speaking with doctors, pharmacists, teachers, financial literacy, or job readiness.
• Documents all case management activities in the electronic health record in a timely manner (day of service), and the entry is complete, accurate, and meets regulatory and agency standards, including identifying the treatment goal in the ISP related to the activity.
• Collects data on residents’ status and needs in areas of financial, environmental, psychosocial, developmental, educational, behavioral, emotional, and mobility.
• Maintains appropriate boundaries and displays respect and courtesy to all internal and external customers and coworkers.
• Demonstrates patience, caring, compassion, and sensitivity toward clients while demonstrating healthy boundaries.
• Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.
• Attends supervision, team and company staff meetings as assigned, and participates in pass-down as assigned by supervisor.
• Maintains mandatory licenses, certificates, and follows drug-free policies of the organization; completes required training in a timely fashion.
• Performs other duties as assigned.

ESSENTIAL SKILLS AND FUNCTIONS:

• Reliable work attendance and ability to work some evening and weekend hours.

• Ability to plan, prioritize and organize workload and establish collaborative work relationships.
• Ability to communicate effectively both in writing and orally.
• Maintain strong client boundaries and demonstrate strong personal health and recovery programs as part of professional self-care.
• Ability to teach life skills and budgeting and to facilitate a group meeting.
• Demonstrated ability to maintain current, accurate and complete documentation in client records.
• Ability to maintain confidentiality of protected health information in accordance with 42 CFR Part 2 (re Alcohol & Drug Records) and 45 CFR (HIPAA).

MINIMUM QUALIFICATIONS:

• At least one year of experience in a substance abuse treatment program; peer certification or experience with pregnant and parenting women are a plus.
• Familiarity with resources for treatment and SUD recovery; an AA or BA in addiction studies is preferred.
• Familiar with Microsoft Word, Office, and Outlook and able to learn electronic health record basics (currently Credible).
• Maintain valid Washington state driver’s license and a driving record that is acceptable to ERC’s insurance carrier (to utilize company car for transporting clients).

PHYSICAL DEMANDS:

The duties requiring sitting, standing, walking, stooping, bending kneeling, crouching, reaching, pushing, pulling, twisting, finger dexterity and sense of touch, ability to grip with hands and fingers, ability to hear voice conversations and to speak, and occasionally lifting up to 25 pounds, often children and babies. Must be able to climb stairs as office is located in a two-story facility without an elevator and individual may visit homes with steps and stairs leading to entrance.
