Job description

The Case Manager is responsible for aiding in the delivery of case management and clinical treatment services and assistance in coordinating program operation.

RESPONSIBILITIES:

  • Ensure that all youth receive services while in care including but not limited to mental health, medical, recreation, education, and other support services
  • Maintain accurate, and current information on foster children in care, while ensuring ORR reporting requirements are met.
  • Coordinate ongoing searches and connection with family or significant adult sponsor (Instead of HV language)
  • Assist the Clinician/Consultant in providing other case management services, including scheduling client’s activities, and making appropriate contacts with referring workers, program staff, etc.
  • Assist the Clinician/Consultant in Case Reviews by sending out invitations, compiling test results, attending, and participating in Case Review Meeting discussions, and writing summaries.
  • Assist the Clinician/Consultant in processing case related information, including profile sheets for new admissions, preliminary work on service plans, follow-up calls for discharged clients, maintaining up-to-date client files, etc.
  • Assist the Clinician/Consultant with linkages with the client’s Goal Worker and other program staff including the Nurses, Wellness Specialists, School Coordinator, etc.; work to keep all service providers informed regarding changes in programming, special needs, etc.
  • Lead or co-lead a milieu group, as requested.
  • May be required to specifically monitor the progress of certain clients, as assigned. This may include work with clients on community living skills.
  • Regular participation in supervision meetings and team meetings for the sharing of ideas, feedback, and department planning.
  • Responsible for quality assurance compliance, including but not limited to participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for each individual served.
  • Responsible for timely Quality Assurance compliance as related to employee’s roles and functions.
  • Other duties as assigned

QUALIFICATIONS:

  • Bi-lingual English and Spanish preferred
  • Bachelor’s degree is required.
  • LBSW or ability to be licensed is preferred.
  • Prior experience working with refugee and/or immigrant populations preferred.
  • Minimum of one year experience as a Case Manager or a Youth Treatment Specialist.
  • Excellent verbal and written communication skills.
  • Federally funded program requiring the following vaccinations:
    • Varicella: two doses of varicella vaccine (at least 4 weeks apart)
      • OR born in the United States before 1980 with a reported history of chickenpox (verified by an adult present at the time of illness)
      • OR titers indicating varicella immunity
      • OR documentation from healthcare provider verifying previous infection.
    • MMR: Two doses of MMR vaccine (at least 4 weeks apart)
      • OR titers indicating immunity to all three diseases (Measles, Mumps, and Rubella)
    • DTaP or DTap
      • Complete primary vaccination series. If primary vaccination series completed in childhood, one DTaP dose in adulthood.
    • Influenza: Single dose annually during flu season.
  • Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal, purpose, and objectives of the agency.

HOURS OF WORK: Monday- Friday, some on-call hours
