Job description

Case Manager – Children’s Program

Location: Kalamazoo, MI, 49008

Reports To: Supervisor of Community-Based Children’s Services

Classification: Part-time, hourly, non-exempt

Job Summary

Assist consumers in the design and implementation of strategies for obtaining services and supports that are goal-oriented and individualized. Services include assessment, planning, linkage, advocacy, coordination and monitoring to assist consumers in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, other services and natural supports developed through the person-centered planning process. Case Management is provided in a responsive, coordinated, effective and efficient manner focusing on process and outcomes.

Tasks/Responsibilities
  • Embrace, support and implement Community Healing Center's vision and values related to diversity, equity, and inclusion.
  • Prepare case histories as needed for use in diagnosis and treatment.
  • Advocate for, link, coordinate and monitor children and family needs with appropriate agency and community program involvement.
  • Provide comprehensive case management services, including consultation, linking families to services, monitoring ongoing progress and coordination between all parties involved. These services may require occasional evening hours to meet the needs of the consumer and family.
  • Attend meetings with Integrated Services of Kalamazoo, school staff and other referral sources to collaborate services.
  • Participate in multi-disciplinary teams as required to assess, plan and review the needs of children and families.
  • Provide transportation, as deemed medically necessary, for consumers and their families, on a limited basis.
  • Maintain required case documentation and record services delivered, in an accurate and timely manner as required by regulating entities, accreditation organizations and management.
  • Provide coordination with and linking to, crisis intervention and hospitalization screening as needed, between the hours of 8:00 am amp; 5:00 pm, Monday through Friday.
  • Adhere to productivity expectations, outlined by CHC.
Education
  • Minimum Bachelor’s Degree in Early Childhood Development, Education, Social Work or other related field, required.
  • Bachelor level Social Worker must have an LLBSW or LBSW.
Job Knowledge/Skills
  • Three years’ experience working in the examination, evaluation and treatment of children and families.
  • Ability to establish and maintain effective therapeutic and working relationships with clients, other staff and community/agency representatives.
  • Reliable transportation and valid driver’s license, required.
  • Direct contact with under-age clients with mental health disorders, abused children and non-offending family members.
  • All staff are required to be screened for tuberculosis and understand that exposure to tuberculosis is possible.
  • Flu and Covid-19 Vaccines, required. Covid-19 booster, highly preferred.
